PRE-PARTICIPATION PHYSICAL EXAM FORMS

All student athletes and any prospective student athlete trying out for a sport are required to submit a Pre-Participation Physical Exam prior to their athletic participation or try-out. 

The physical exam must be within (6) months of when their respected sport begins fall practices.  Please note that this is per the NCAA Medical Handbook and not a Bentley University policy, therefore no exceptions can be made. 

In addition, a Sickle Cell test result must also be submitted.

The Pre-participation Physical Exam Form may be obtained by clicking on the link below.  You cannot fill out the forms online, you must print them off.

• Bentley University Medical History and Orthopedic Physical Exam Form

This form is different than the Bentley Health Services forms and cannot be interchanged. 

Freshmen, returning athletes and fall transfers must submit their forms by July 31, while try-outs must submit their forms (1) week prior to their try-out.  All completed forms must be submitted to the Bentley University Sports Medicine Department via mail or fax.
