PRE-PARTICIPATION PHYSICAL EXAM FORMS 

According to the NCAA Medical Handbook, all incoming student athletes, and any prospective student athlete trying out for a team, must submit the Pre-Participation Physical Exam and Sickle Cell test results prior to any athletic participation. All returning student-athletes must fill out the Pre-Participation Physical Exam Form. The link is below.

Any student athletes who are currently taking any banned ADHD medication (Ritalin, Adderall) must also submit appropriate paperwork to fulfill the NCAA Drug testing requirements.

These forms are different than the Bentley Health Services forms and cannot be interchanged.
